What requires a Prescription?
A prescription is required for the purchase of the ReBuilder® 300 or ReBuilder® 2407.
Submitting Your Prescription
Please fax your prescription to (304) 725-4915.
If your Rx is already on file with us, just let us know in your notes with payment. If an item is purchased and we do not have your Rx on file, we will reach out to you by phone and email until we are able to receive your Rx so that your order may be shipped.
If you have any questions about obtaining a prescription, please call us at (877) 717-5487. Our office is open Monday through Friday 8:00am-6:00pm EST.
Sample format and codes that your physician will need are as follows:
- Diagnosis: Chronic Intractable Pain (DX Code – G89.4)
- Other Possible Diagnosis Codes: Other Chronic Pain (DX Code – G89.29) and Unspecified Idiopathic Peripheral Neuropathy (DX Code – G60.9 DX Code – G90.09)
- Sample Rx Language: ReBuilder® Model 300 for treatment of chronic pain, ReBuilder® Model 2407 for treatment of chronic pain.
- Description for ReBuilder® is: TENS/EMS, ReBuilder®, brand specific for chronic pain (No Generic Substitutions)
